Are you a young person living in Ontario who has lived experience of mental health/substance use challenges -- or are you the caregiver/parent of one? Or do you provide services to youth and their families?

Would you like to participate in our research project?

The purpose of the survey (link below) is to find out what types of services you would like to see in an integrated service hub — a one-stop-shop for youth mental health services!

Filling out this survey will take about 25 minutes to complete and you will be entered into a draw for one of forty gift cards.

Who should participate in this study?

Residents of Ontario, youth (aged 14-29) with lived experience with mental health and/or addictions, caregivers of youth (aged 14-29) with lived experience with mental health and/or addictions, service providers/agency representatives with experience supporting youth (aged 14-29) with mental health and/or addictions.

Participation is completely voluntary. You do not have to answer every question and can stop at any time with no penalty. The online survey can be completed at a time that is convenient to you.
