Free webinar for families: Empowering family caregivers to improve health care communication

Families play a key role in supporting adults with developmental disabilities (DD) to manage their health. How can parents and siblings support their family member with a developmental disability to get the best health care possible?

This webinar will focus on two key communication tools, About My Health and My Health Care Visit, and how using these tools can really make a difference. This webinar is part of the Azrieli Family Education Series, and can be attended on-line or in-person.

Health Care Access Research and Developmental Disabilities (H-CARDD) is a research program that aims to enhance the overall health and well-being of people with developmental disabilities (DD) through improved health care policy and services. For this project, H-CARDD partnered with Vita Community Living Services (CLS) and the Developmental Disabilities Primary Care Program at Surrey Place.

This work was funded by the Ontario government’s Employment and Modernization Fund, with knowledge translation activities funded by an EMPOWER grant through the Ontario SPOR SUPPORT Unit, which is supported by the Canadian Institutes of Health Research and the Province of Ontario.
