Recovery aftercare pilot program extension

Project description

This is one of two projects (one is an extension of previous DTFP work while the other is a new project) that will be supported and are in line with DTFP objectives and investment areas.

The Recovery Aftercare Pilot Program Extension will have a lead in the department to oversee the continuation of outcome monitoring for adult addictions clients, as well as expansion of the program to youth. This program will conduct baseline and follow-up surveys with adults and youth, in addition to post-treatment follow-up that will link clients back into treatment if required. Linking clients back into treatment is considered best practice in addictions. Client outcome data will contribute to evidence-based program planning and quality improvement for both youth and adult addictions programs in all regions of the province.

A Knowledge Exchange Facilitator, located in Central Regional Health Authority, will assist with the development and implementation of both projects.
